N2 - A prototype of web-based medical equipment maintenance system is an application which can be used as a tool to help electro medical technicians in maintaining and calibrating medical equipment. This research was aimed at establishing a prototype for computerized maintenance and management system (CMMS) implemented at the Hospital Infrastructure and Facility Maintenance Unit. This research was a post-test only experimental research. The analysis of development and design of the system was based on system development life cycle, with waterfall model. The system was developed by using PHP as the programming language and MySQL as the database, installed in an Apache Server. Meanwhile, the system was designed by using HTML 5, CSS, JavaScript, and Bootstrap CSS Framework. The result of testing conducted by three programmers shows that the system usability aspect calculated with Likert scale was 88%. Therefore, it can be concluded that the prototype of web-based medical equipment maintenance system is very easy to use, and it can be used to prepare documents required for accreditation and re-accreditation of hospitals based on Facility and Safety Management (MFK) 8 and 8.1.
