T1 - The Efficacy of Partial Splenic Embolization in Patients with Hypersplenism: An Evidence- Based Case Report

N2 - Introduction Splenectomy is the main therapeutic choice in treating hypersplenism due to liver cirrhosis, yet the essential function of spleen as a site where immunecells are produced and differentiated would be completelely diminished if someone undergo splenectomy. One of the alternative methods in treating hypersplenism is partial splenic embolization (PSE),and it could preserve spleen function. This study isaimed to investigate the efficacy of PSE in treatinghypersplenism. Methods This was an evidence-based case report. It was conducted at the Division of Vascular Surgery, Department of Surgery, Faculty of Medicine – Universitas Indonesia, Cipto Mangunkusumo Hospital. Thestudy was conducted during the period of July toAugust 2017. A literature search was brought out through reliable databases on PubMed, Science Direct, Cochrane Library, Springer Link, NICE database, and TRIP database. The inclusion criteria systematic review, randomized-clinical trial (RCT), orcohort studies, studies conducted on humans, and written in English. Results Four articles were found to be eligible in this study. All of these studies suggested that groups that underwent PSE had significantly better outcome compared to those who underwent splenectomy. Conclusion Several studies with level of evidence of one, two,and three, revealed that partial splenic embolization(PSE) were beneficial in patients with hypersplenism due to hepatic cirrhosis. Further studies regarding the long-term outcomes of PSE are required inorder to determine the recurrence rate and the costeffectiveness aspect of PSE.
