Abstract
Facial clefts are extremely rare congenital anomalies. There are various types of clefts, and classifications are required for their description. Nasal anomalies are rare and, currently, there are no standard methods for the management of such defects. The number of operations and types of surgical intervention vary widely according to the pattern and complexity of the deformity. The aim of this case report is to formulate a standardized procedure for there pair of the nasal component of Tessier no. 3 clefts. A nasal dorsum advanced rotational flap was used to close the cleft of ala nasi during the first-stage surgery. Second-stage surgery involved an osteotomy and septoplasty to obtain a better shape for the nasal dorsum. During the third operation, we reconstructed the ala nasi to obtain improved symmetry. The nasal dorsum rotation flap showed good healing over the lateral surface of the nose and in the subciliary region. The closure of the nasal defect was accomplished using a composite downward rotation flap of the ala based on the lateral cleft wall. An advanced rotation-transposition flap was used in the management of a Tessier no 3cleft. We also recommend the use of a superiorly based transposition for extensive lateral nasal wall involvement. In the present case, we performed osteotomy and septoplasty to obtain an improved shape for the nasal dorsum. Although the symmetry between the two nostrils was not ideal, it was acceptable.
