Fournier gangrene in an Indonesian male patient with uncontrolled type 2 diabetes mellitus: A case report

Abstract

Fournier gangrene (FG) is a form of necrotizing fasciitis affecting the genital, perianal, and perineal regions. It is a very rare disease that affects immunocompromized individuals. The prevalence of FG in Asia, specifically in Indonesia, remains unknown. We report a 42-year-old Indonesian male patient with FG and an unknown history of diabetes mellitus (DM), who presented at the emergency room of Jakarta Medical Center Hospital in Jakarta, Indonesia. The patient had a chief complaint of left scrotal lump for 1 week prior to hospital admission. The patient had random blood glucose levels of 448 mg/dL upon hospital admission. He was diagnosed with FG and uncontrolled type 2 DM. Physical and laboratory examinations showed a Fournier Gangrene Severity Index score of +3. The ultrasonography examination revealed exudate in the left scrotum. The patient received antibiotic and surgical therapy (necrosectomy and debridement). On the following day, the patient had an improved clinical outcome and controlled random blood glucose.
