Abstract
Introduction: It is believed that the smile represents personality and appearance. The common cause of excessive gingival display is altered passive eruption (APE), which was resulted from incomplete natural migration of the gingiva during tooth eruption. Case report: A 30-year-old female was referred from Orthodontist in private practice. At the first visit, the patient chief complaint was her tooth arrangement, and asked any treatment to help her in order to minimize her gummy emergence when she was smiling. From the clinical examination, there was an excessive gum appearance of about 5 millimetres (mm) in her upper jaw. This case showed the unequal proportion of the teeth, including the square shape of central incisive, the proportion of lateral incisive and canine. The ratio of intra-tooth relationship for the central incisive was 8 mm:8 mm (height :width). In addition, the gingival zenith level was also not ideal. The management of this case was divided into the preparation phase, surgery phase and follow-up phase. In the preparation phase, we examined the biological width, anatomical consideration and esthetic consideration. In the surgery phase, we prepared for crown lengthening procedure with gingivectomy and bone reduction by 2 - 3 mm. From the result, the patient was very satisfied with her new tooth shape and smile. Conclusion: Crown lengthening procedure is one of the alternative treatments for altered passive eruption cases. Bone reduction estimation was calculated by the evaluation of biological width, anatomical and esthetic consideration.
