Abstract
Introduction: Occupational safety and health are important aspects in the chemical industry to prevent process accidents that can endanger human lives and the surrounding environment Objective: To identify the factors that cause process accidents in the chemical industry. This research also aims to understand the root causes and factors that lead to safety-threatening incidents in the chemical industry sector. Method: To identify the factors that cause process accidents in the chemical industry. This research also aims to understand the root causes and factors that lead to safety-threatening incidents in the chemical industry sector. Result: The results of the analysis show that equipment failure is the most common cause of accidents with a percentage of 35%, followed by non-compliance with procedures with a percentage of 25%, and material failure with a percentage of 14%. Other causes such as deficiencies in equipment design, natural factors, presence of ignition sources, and lack of hazard identification each account for lower percentages Conclusion: It is important for industry to improve the maintenance and supervision of equipment to reduce the risk of accidents. In addition, enforcing strict safety procedures and understanding the factors that cause material failure is also key in preventing incidents that could endanger work safety.
